Required Documents
1- Cover Letter
2- Completed Application for Temporary Resident Visa (IMM 5257)
3- Completed Family Information Form (IMM 5645), if applicable
4- Completed Schedule 1 - Application for Temporary Resident Visa (IMM 5257 - Schedule 1), if applicable
5- Statutory Declaration of Common-Law Union (IMM 5409), if applicable
6- Use of a Representative form (IMM 5476), if applicable
7- Application processing fee and biometric fee
8- Valid passport (there must be one completely blank page other than the last page which is available in every passport)
9- Two recent photographs taken within the last 6 months and meeting the requirements of the visa application photograph specifications. Write the name and date of birth of the applicant on the back side of two photographs
